PUBLIC NOTICE
The Joint Commission on Accreditation of Healthcare Organizations will conduct an accreditation survey of LYONS COSMETIC & LASER SURGERY CENTER, MICHAEL B. LYONS, M.D., F.A.C.S. 3664 College Lane S. Mobile, Al. 36608.
The purpose of the survey is to evaluate the practice's compliance with nationally established Joint Commission standards. They survey results will be used to determine whether, and the conditions under which, accreditation should be awarded to the practice.
Anyone believing that they have pertinent and valid information about quality of care issues and the safety of the environment in which care is provided may request an interview with the representative at the time of survey. Requests must be made in writing, and should be sent to the Joint Commission no later than five working days before the survey date. The request must also indicate the nature of the information to be provided at the interview. Such requests should be addressed to:

The Joint Commission will acknowledge such requests in writing or by telephone and will inform the practice of the request for any interview. The practice will, in turn, notify the interviewee of the date, time, and place for the meeting.
This notice is posted in accordance with the Joint Commission's requirements and may not be removed before the survey is completed.
